This is a PDF template
It works best in the FREE desktop version of Adobe Reader. You can use your mobile and other PDF editors, however the font alignment may be off.
How it works
- Download the file (and more detailed Instructions) below
- Open in Adobe Reader to avoid editing/font issues
- Click on a shaded box, highlight my sample text & start typing
- Use the top left File menu options to Save & Print. Print at 100% actual size
